Alchemical ingredient. The sweetroll is a staple for many of Bethesda's games, and is sort of an ongoing joke.

Location: In houses and buildings. Most inns sell them. There's a sweetroll/sweetcake baker in Skingrad. If you need a sweetroll to give to Fimmion for 'The Coming Storm' quest, you can find one in Rendil Drarara's house in Bliss. He stands near a clutch of sweetrolls on a shelf, usually with his back to them, so you can steal one without penalty.

Occasionally, in Skingrad, if you ask an NPC about rumors, they will tell you of Salmo's sweetrolls. Then they will say "One time I took a sweetroll into an allyway to eat when three thugs jumped out at me. So I took the sweetroll and...". This is a reference to The Elder Scrolls 3: Morrowind where, at the beginning of the game, one of the skill-determining questions the player is asked is "The local baker gives you sweetroll to enjoy. Delighted, you take the sweetroll into a back ally to enjoy, only to be intercepted by three thugs. The leader demands that you give him the sweetroll, or they will beat you and take it. What do you do?"
In Fallout 3, this scene is acted out. At the player's 10th birthday party, a kindly old lady gives you a sweetroll. When the birthday cake is ruined, the local bully attempts to coerce you into giving him the sweetroll. Most responses will result in him punching the player until security steps in.
